Apply for Assistance

Thank you for considering the FACE Foundation to help you with your pet’s health care needs.  We hope we are able to be of assistance to you. We have established the procedures listed below to help guide you as you decide if FACE will work for you and your pet. Please review our procedures carefully.

Procedures for Applying for FACE Funding


Who Qualifies for FACE Funding?

  • FACE provides financial assistance for emergency veterinary surgeries and procedures directly to veterinary hospitals for pet owners facing financial hardship.
  • Your pet must be diagnosed with an immediately life-threatening condition that has a good to excellent prognosis if treated.
  • FACE can ONLY provide assistance to pet owners residing in San Diego County.
  • Please note, FACE cannot provide assistance for:
    • Initial veterinary exams or diagnostics.
    • Services already rendered or reimbursements for costs incurred prior to FACE approval.
    • Routine veterinary care (i.e.dental work, vaccinations, etc.), or for conditions requiring ongoing treatment (i.e. cancer, diabetes, etc.)

 

How Do I Apply for FACE Funding?

Once your pet has been examined by a veterinarian who diagnoses your pet with an immediately life-threatening condition with a good prognosis if treated, complete the following steps:

  1. All adults in the household must apply for Care Credit (www.carecredit.com) and provide results of the application. If you are approved for credit you are required to use your available credit before receiving funds from FACE.
  2. Complete a Request for Financial Assistance Form. The owner completes the top portion, and the treating veterinarian completes the box underneath the owners’ signature. You can download this document for printing by clicking the orange link below.*
  3. Provide proof of financial income for all adults in the household; including but not limited to: 1040 tax forms, 3 months’ recent bank statements, unemployment or Social Security Income information, etc.
  4. The treating veterinary hospital must provide complete and accurate medical records and an estimate for the procedure.
  5. Please call us at (858) 450-3223 once you are ready to submit your paperwork, or if you have any questions.
  6. All information can be faxed to (858) 457-3223, or emailed to grants@face4pets.org. We highly recommend you email photos of your pet with your application.
  7. Once your completed application with supporting paperwork is received, it will be sent to our Granting Committee for review. The Granting Committee will make the final decision on approval or denial of your application, and will decide the funding amount. FACE typically has a response within 24 hours of submitting an application to our Granting Committee.

Notes on Receiving FACE Funding:
  • All recipients of FACE funding agree to allow FACE to use their story, photos and video (if taken).
  • Recipients are asked to do whatever possible to help FACE save other family pets. This includes sending an e-mail to family and friends, volunteering when possible and participating in special events.
  • All animals that receive FACE funding must be spayed or neutered. If an animal is not spayed/neutered at the time funding is given, it will be sterilized at the time of the procedure if the treating veterinarian determines that it will not jeopardize the health of the animal.  If sterilization cannot be performed at the time of initial treatment, the owner will agree to have their animal sterilized within a set time frame determined by the treating veterinarian (based on the health of the animal). This will be performed at the owner’s expense and is not covered by the initial grant.
